Victim of Lac des Iles fatal incident identified

THUNDER BAY -- The worker killed in a workplace incident at the Lac des Iles mine last week has been identified. Pascal Goulet, 38, died on July 10 after he was struck by a piece of ore outside of his loader, North American Palladium Ltd.

Pascal Goulet, 38, died on July 10 after he was struck by a piece of ore outside of his loader, North American Palladium Ltd. revealed in a statement on Monday.

Goulet was working more than 800 feet underground when he was pinned under the rock and was pronounced dead at the scene.

The provincial Ministry of Labour issued two stop work orders, one each for the location and the equipment involved in the incident as part of their initial investigation.

The ministry also issued an order to examine the grounds for dangers and hazards as well as an order to establish procedures to ensure operators and other workers are in safe locations when equipment is used or moved.

"We are profoundly saddened by this tragic accident and we extend our deepest condolences to Mr. Goulet's family, friends and colleagues," president and CEO Phil du Toit is quoted in a release on the company’s website.

The company statement said Goulet is survived by his wife and two daughters.
