•Working as an integral member of the Alzheimer Society team, the Community Outreach Support Worker will work full time supporting a variety of needs for persons living with dementia in the community. The ideal candidate must be highly motivated, with a passion for persons living with dementia.
•The Community Outreach Support Worker will provide activation in a safe and competent manner within their scope of practice.
•Develop and maintain an active client caseload
Knowledge/Skills/Abilities:
1. Post-secondary degree or diploma in Recreation Therapy or graduate of an approved Personal Support Worker program. Other health disciplines may be considered given appropriate combination of education and experience.
2. Knowledge of Alzheimer’s disease and related dementias (ADRD).
3. Demonstrated knowledge and best practice in dementia, delirium and their effect on persons, care partners and extended family.
4. Experience working with persons living with dementia and their care partners.
5. Complete assessments of potential clients and determine eligibility and appropriateness for the Community Outreach Program
6. Experience designing individualized recreational programs to support individual needs, as well as facilitating group activities.
7. Knowledge of community health care agencies.
8. Current Standard First Aid/CPR certification.
9. Ability to work in conjunction with volunteers, staff, and community partners.
10. Competency with Microsoft Office applications.
11. Valid driver’s license, clean driver’s abstract and regular access to a vehicle. The vehicle must be insured for work purposes.
Responsibilities:
1. Conduct in-home programs with persons living with dementia on a daily basis.
2. Assist persons living with dementia with activities of daily living when required and support mobility needs.
3. Plan and organize activation activities for persons living with dementia.
4. Evaluate the effectiveness of programming for clients.
5. Provide education and support to Persons Living with Dementia and their care partners.
6. Act as a resource for inquiries from the general public regarding recreation/activation.
7. Ability to respond effectively in crisis situations
8. Ensure that client consents, privacy and confidentiality are maintained in compliance with legislation, professional standards/regulations and internal policies.
9. Experience with an electronic documentation system and daily client documentation is a necessity.

11. Ability to take initiative and be resourceful
12. Ability to work independently in client homes
13. Excellent problem-solving abilities.
14. Adhere to program standards and Alzheimer Society policies and procedures.
15. Assume responsibility for advancement in dementia knowledge and ongoing professional development.
16. Participate in fundraising and other team development activities.
17. Assume other functions as may be assigned by the Executive Director/Manager Clinical Services
18. Bilingualism (English/French) is an asset.
