Pascal Goulet, 38, was killed instantly on July 10, 2014 while on shift underground at the mine when he was struck and crushed by a run of muck or rock outside his loader. The company was fined $300,000 Tuesday in provincial offences court, found guilty of failing to ensure that written safety precautions and procedures were established and followed to prevent a worker from being outside of a loader while ahead of the safe-limit line.

On Feb. 22, 2014 another worker was trapped in an excavator after a larger amount of material fell on it. The worker was rescued by mine staff after two hours and suffered leg and hand injuries. The company was fined $65,000 for that incident Tuesday, guilty of failing to ensure that a stockpile of unconsolidated material was made safe before a worker was allowed to work close or on top of the stockpile.

Both charges come under Mining and Mining Plants Regulation. 

The Ministry of Labour also issued a number of order to the company. A 25 per cent victim surcharge fine was also handed out.
